Notes on the medical form.
We are not asking for details of either current or past medical history, what we need to know is if you have a medical condition that MAY need a rapid intervention from the medical service, and where you keep any medical device.
i.e
One of the members has a heart condition and has for sometime to the extent that he had a massive heart attack in 2007, the technical term is an MI (myocardial infarction) which is a serious medical emergency in which the supply of blood to the heart is suddenly blocked and the heart may stop.
Today he has always carries a spray (GTN) that helps' relieve the pains in the chest should he use it more than 3 times over a 15 minute period we are to call an ambulance immediately on the 3rd spray, as he might be having another, his GTN spray can be found in his left pocket.
From this information we know
1. if he's having problems why and where we can find his spray.
2. if he can't talk but sign how many times he used his spray.
